Please complete the form below before your pet’s procedure.

Pre-Surgical Instructions:

- We will email you two authorization forms and a financial estimate. Please fill these out, sign them and bring them with you at drop off
- Drop off is between 8:00AM-8:15AM.
- **Please encourage your pet to have a bowel movement before coming in.**
- NO FOOD/WATER AFTER 10:00PM the night before the procedure.
- If you have an Elizabethan Collar (cone collar), please bring it with you when you are dropping your pet off.
- If your pet takes medication in the morning, please ask the surgical technician or the veterinarian for instructions regarding whether or not to give them.

Post-Surgical Instructions:

- Your surgical doctor or technician will give a call when the procedure is finished, and your pet is awake.
- Pick up is typically between 3:30pm-5:30pm. We will have you schedule a discharge appointment.
- You will receive Home Care Instructions at discharge that will have feeding, exercise, medicine and recheck instructions for your pet.

We believe medical insurance for pets is an important part of responsible pet ownership. It can help pet owners budget for any unexpected illnesses or injuries that their pets may face over their lifetime. We are happy to work with any provider you may currently be insured with. If you are not insured, we recommend that you look into it and see if it’s right for you. We have materials available in our hospital for you to learn more about coverage options.
